The Onondaga County Health Department has asked the Fayetteville-Manlius School District, and other school districts within the county, to share with families a survey it will use to inform its next Community Health Assessment and Improvement Plan.
Every three years, the health department produces an assessment and improvement plan in partnership with local hospitals. To inform the process, the health department distributes a community survey to hear directly from county residents their perspectives of the community as it relates to health and healthcare.
Given the challenges of the past two years, Onondaga County Health Department Commissioner Indu Gupta has indicated that it’s more important than ever that they understand the community’s perspectives and priorities. The survey takes about 10 minutes to complete, and respondents can enter to win one of five $50 Target gift cards. The survey will be open until May 13.
