To better serve families, students and educators, New York State Education Department (NYSED) is asking parents, guardians and caregivers to provide feedback about their student’s home access to technology and the internet.
A nine-question digital equity survey is now available for each Fayetteville Manlius School District student within their SchoolTool account. Parents, guardians and caregivers can also complete the survey using the SchoolTool app.
This is the second year the survey is being administered. NYSED will use survey feedback to determine school districts’ technology funding needs.
The survey should be completed as soon as possible. Families only need to complete the survey once.
Web version – how to access, complete and submit the survey:
- Log into SchoolTool.
Your username is the email address that you provided to F-M. If you do not know your account information, please select “forgot password.” If you still cannot log in, call the help desk at 315-692-1969. - Select the User Defined tab.
- Select the “Plus” symbol.
- Fill out the survey.
For question 1, select “Yes” even if your child declined a device.
For question 2, K-1 uses an iPad, 2-12 uses a Chromebook.
For question 3, if your child does not use the iPad or Chromebook provided by the school when at home, select “Personal” or “None.” - After all answers have been entered, select the disc icon to save (submit) the survey.
App version – how to access, complete and submit the survey:
- Log into SchoolTool.
If you have not installed the SchoolTool app, you can do so at the app store. Your username is the email address that you provided to F-M. If you do not know your account information, please select “forgot password.” If you still cannot log in, call the help desk at 315-692-1969. After successfully installing the app and logging in, type in the survey web link. - Tap on the forms panel.
You’ll see a list of forms that are currently assigned to your students. If a form is only assigned to one student, you’ll see that student’s name. If the same form is assigned to multiple students, you’ll see all of their names listed in a dropdown. If responses have already been submitted for a student, that student’s name will be displayed in blue, while students whose forms are still due are displayed in red. - Tap on a student’s name under the form you want to complete.
You can submit the same answers for multiple students in a single operation. It doesn’t matter which student you start with; you’ll be prompted to select additional students before you submit your response. The NYS Digital Access Survey consists of nine questions. You must select an answer from the dropdown for each question in order to complete your response. - Fill out the survey.

For question 3, if your child does not use the iPad or Chromebook provided by the school when at home, select “Personal” or “None.” - Complete the form as appropriate and click “SAVE & CONTINUE” in the bottom right.
If the form was assigned to multiple students, you can select the other students here to submit identical responses for each student. Choose the student(s) for whom you wish to submit responses and confirm the submission. - Once the response has been saved, you’ll see a confirmation message.
If you need to change your responses, you can click on the forms panel and repeat the steps above to re-submit your answers.
