Health screening tool for students launches Oct. 10

The Fayetteville-Manlius School District is adding another layer of precaution to its health and safety protocols for in-person instruction. 

On Oct. 10, parents and guardians will begin receiving a weekly email via SchoolMessenger asking them to complete by noon on Mondays an online health attestation for each child in their family who is participating in in-person instruction. 

This weekly COVID-19 questionnaire, which will be emailed to families on Saturdays, is part of the New York State Department of Health’s requirements for reopening schools. The questionnaire must be completed once per week prior to the first time a student arrives on school grounds.  

This health and safety measure will be in addition to the daily temperature checks that are taken at school.

Parents and guardians of cohort A and B students must complete the questionnaire each week. Families of cohort C students who are participating in fully-remote instruction do not need to complete the health screenings weekly, but they must submit the form by noon on Monday if their child will be visiting school grounds for any reason that week, such as participating in interscholastic athletics.
